Liability Waiver Each family must sign prior to participating in an activity or event. This form is good for one year.
Background Check Each adult volunteering or teaching for RISE Resource Center must complete a background check. For your ease we offer these online. Payment of $10 is due to RISE Resource Center for each background check submitted.
